Welcome to Access PathLab (operated by Access Pathlab India Pvt. Ltd.). By accessing or using our website (accesspathlab.com), mobile application, or any of our services including lab testing, home sample collection, and online booking, you agree to be bound by these Terms and Conditions. Please read them carefully before using our services. These terms constitute a legally binding agreement between you ("User", "Patient", or "Customer") and Access Pathlab India Pvt. Ltd. ("Company", "We", "Us", or "Access PathLab"). If you do not agree with any part of these terms, please do not use our services.

Access PathLab has been providing diagnostic and pathology services since 2011. Our services include: • Laboratory diagnostic testing (blood tests, urine tests, and other clinical investigations) • Health checkup packages and profiles • Home sample collection service at your doorstep • Online booking and report delivery • Prescription-based test recommendations We are committed to providing accurate, reliable, and timely diagnostic services through our high-quality laboratory processes.

To use our services, you must be at least 18 years of age. Minors may avail our services only under the supervision of a parent or legal guardian who accepts these Terms on their behalf. By creating an account or placing a booking, you represent that: • You are at least 18 years old or have parental/guardian consent • The information you provide is accurate and complete • You have the authority to enter into this agreement

To access certain features such as online booking, report access, and order tracking, you may need to create an account. You are responsible for: • Providing accurate and current information during registration • Maintaining the confidentiality of your account credentials • All activities that occur under your account • Notifying us immediately of any unauthorized use of your account We reserve the right to suspend or terminate accounts that violate these terms or contain false information.

When you place a booking through our website or application: • You agree to provide accurate patient details including name, age, gender, phone number, and address • Test prices displayed are inclusive of applicable taxes unless stated otherwise • We reserve the right to modify prices at any time without prior notice; however, confirmed bookings will be honored at the booked price • Booking confirmation is subject to service availability in your area • For home collection, you must ensure the patient is available at the provided address during the selected time slot • We may require a valid prescription for certain tests as per regulatory requirements A booking is considered confirmed only after you receive a confirmation notification via SMS, WhatsApp, or email.

Our home sample collection service is subject to the following conditions: • Service availability depends on your location and pin code; we currently serve select cities and areas • Our trained phlebotomist will visit your provided address during the selected time slot • You must ensure a safe, clean, and well-lit area for sample collection • The patient must carry a valid photo ID for verification purposes • An OTP verification may be required before sample collection for security • If the patient is unavailable or the address is incorrect, a revisit fee may apply • We reserve the right to refuse collection if conditions are deemed unsafe for our staff or the patient • Fasting requirements and other preparation instructions must be followed as advised Sample collection timing, phlebotomist assignment, and route planning are managed by our operations team. Live tracking of the phlebotomist may be available for eligible orders.
